First Time User Form

If you're a first time user please read this 'colour eye accessory assessment form':

I do not suffer for any of the following:

- Any eye infections (e.g. Conjuctivitis) 

- Any corneal condition (e.g. Keratitis)

- Any allergic eye conditions (e.g. Hay fever)

- Any chronic eye conditions (e.g. Glaucoma)

I intend to use FUNKY VISION Colour Eye Accessories for cosmetic colour change purposes only and not to correct any defects of sight or refractive error.

I have received clear instructions and guidance for the safe wearing, removal and care of my new Colour Eye Accessories.

I have been advised to immediately remove my Colour Eye Accessories if I experience any undue irritation, discomfort, excess watering or redness of the eye.

I have received details of FUNKY VISION helpline number: 01507 605777.

I understand the need for regular periodic check-ups with an eye care practitioner.

I am over the age of 16.

I understand and acknowledge the FUNKY VISION, Colour Eye Accessories are not classified as medical devices and are governed by the EU Cosmetics Directive; however, in the clinical opinion of the company's qualified ophthalmologist, they are only considered suitable for those with normal healthy eyes, who can confirm ALL 7 statements above to be true and accurate.

All lenses ordered include a instruction for use leaflet. Reading the terms and conditions confirms that you will read this information before use.

-       Eye accessories are intended for occasional use only.

-       These are not to be worn by people under the age of 16.

-       Never swap lenses with another person.

-       Ensure you use a suitable multi purpose soft contact lens solution.

-       Once opened the lenses have a life span as indicated on the packet.

-       Eye accessories are suitable for normal, healthy eyes. If you are in doubt as to whether these eye accessories are suitable for you, then please see a medical or eyecare practioner.

  • DO NOT wear if your eye accessories appear to be damaged.
  • DO NOT share your eye accessories with anyone else.
  • DO NOT let hairspray or any other cosmetic aerosols come into contact with your  eye accessories.
  • DO NOT participate in any water sports whilst wearing
  • DO NOT wear your eye accessories when using eye medication
  • DO NOT use tap water, saliva or any household cleaning agents to clean or wet your eye accessories or storage case.
  • ALWAYS remove your eye accessories before you go to bed
  • ALWAYS put your eye accessories in before applying make-up
  • ALWAYS take out eye accessories before removing make-up
  • ALWAYS replace your eye accessories if your eyes get red or irritated or if you have any discomfort or pain, and seek help from a qualified eyecare professional.

Important Information

Please note that these Eye Accessories are not intended to correct ant defect of sight and are intended only for cosmetic colour change.

If you suffer from any of the following, then you MUST NOT wear these eye accessories:

  • Corneal dystrophies
  • Corneal ulcers
  • Inflammation of the comea, e.g. keratitis
  • Inflammation of the eye, e.g. iritis, uveitis
  • Allergic eye conditions
  • Any pre-existing eye condition

If you are in any doubt, please seek further advice from medical or eyecare practitioner.

Eye Accessories are suitable for normal, healthy eyes. If you are in any doubt as to whether these eye accessories are suitable for you, then please consult a medical or eyecare practitioner.

Fitting and Aftercare

It is recommended that you consult your eyecare practitioner to ensure suitability and for practical advice on wearing and removing, (consultation fees may be charged).

Always have regular check-ups and follow the advice of your medical or eyecare practitioner.

If you feel any unexplained discomfort, watering, vision change or redness, remove immediately and seek help from a qualified eyecare practitioner.
